NOTE: NEW MSHSAA PHYSICAL FORMS
The Missouri State High School Activities Association – MSHSAA – has changed the required physical form for participation in athletics and activities effective 2/1/11.
As part of this new form parents and student athletes must sign a statement that they have read the information on concussions. Below is the new form and the Concussion Packet and Concussion Form.
Any student athlete who has already turned in a physical dated after 2/1/11 on the previous MSHSAA Physical form must complete and turn in the statement page that they have read the concussion packet information. Physicals without the signed concussion statement portion of the physical form will be considered incomplete until the Concussion form is turned in to your high school/middle school activities office.
