Welcome to Lee’s Summit North High School!  To begin the enrollment process at LSN, we must have the following documentation. 

PLEASE UNDERSTAND THAT AN APPOINTMENT WITH THE COUNSELOR CANNOT BE MADE FOR YOU AND YOUR STUDENT UNTIL YOU HAVE ALL DOCUMENTS LISTED BELOW.

• #1: Proof of Residency:
  
• Current, original utility bill (electric, gas (not propane) or water) within the last 30 days.  The bill must show the resident’s name and the service address.  We do not accept final or disconnect notices.  We will accept on-line payment information only from the utility company (not a third party pay service) and must include all the information listed above.
• Rental/lease agreement including dates of agreement, name of lessee and lessor, address of lessee and lessor, phone number of lessee and lessor and signature of lessee and lessor.
• Sales contract with the closing and possession date and all signatures.

 

• #2: Proof of Age:
  
• An official birth certificate or other official document with the following information:  child’s legal name, birth date and parent/parents’ name.

 

• #3: Immunization Records:
  
• According to state law, documentation of immunizations is required before a student can be enrolled.  A copy of a complete official immunization record from the former school, health department, or doctor’s office must be provided.

 

• #4: Academic Records:
  
• Transcript
• Withdrawal Grades

 

• #5: Special Education Records (if applicable):
  
• IEP and Evaluation Report

 

• #6: New Student Packet:
  
• New student packet (which includes several forms that must be completed by parent and student before appointment will be scheduled). These forms can all be accessed on this page or a packet can be picked up from the guidance office.

 

After this information has been acquired, please contact the Guidance office at 816-986-3003 to set up an appointment and meet with the counselor.  (Please note that the Guidance office is closed during the summer months.  If you call during that time, please leave a message and we will return your call.)
